Makeup Consent: As part of the Spa experience, age-appropriate hypoallergenic makeup and spa treatment products will be applied. This includes but is not limited to lip gloss and eye shadow. I acknowledge that any beauty or “spa treatments are for entertainment, and Royal Divas Spa Bus are not licensed cosmetology professionals. Royal Divas Spa Bus will not cut, file, trim, push back cuticles or do any services provided at a certified nail salon. Royal Divas Spa Bus is an event planning company, and the spa party themes are for entertainment only.

Photo and Social Media Release Form for Minors: I give Royal Divas Spa Bus permission to use my child’s likeness, image, voice, and/or appearances as such that may be embodied in any pictures, photos, video recordings, digital images, and the like taken or made on behalf of Royal Divas Spa Bus, party activities. I agree That Royal Divas Spa Bus has complete ownership of such pictures, etc., including the entire copyright, and may use them for any purpose consistent with Royal Divas Spa Bus Parties. These uses include but are not limited to, reprints, publications, advertisements, social media, and any promotional materials in any medium now known or later developed, including the Internet. I acknowledge that I will not receive any compensation for using such pictures, etc., and hereby release Royal Divas Spa Bus from any claims connected with such use. I have read and understood this consent and release.

Food Allergy Waiver: Your child will be participating in entertaining spa-like activities. Some of these activities involve food products as part of the experience. We know many children have allergies, some of which can be life-threatening. We take many precautions to ensure that all the children are safe. Royal Divas Spa Bus will not be held responsible for any allergic reaction or life-threatening incident, including death, that may occur. If your child has any known food allergies or intolerances preventing her from participating in these activities, we will provide an alternate option at your request.

Coronavirus/Covid-19: I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further believe that Royal Divas Spa Bus has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I further acknowledge that Royal Divas Spa Bus can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, Party staff and other guest and their families. I voluntarily seek services provided by Royal Divas Spa Bus and acknowledge that I am increasing my risk of exposure to the Coronavirus/COVID-19. I recognize that my family and I must comply with all set procedures to reduce the spread while attending the Royal Divas Spa Bus event.

Liability: We Strictly provide entertainment. We cannot be held responsible for accidental damage caused by your child. Royal Divas Spa Bus assumes no responsibility or liability for accidents or damage caused during the party. In the event of damage or loss or our inventory and/or damage or loss to the party bus, the responsibility lies entirely on the client and/or guardian. Royal Divas Spa Bus is not responsible for allergic reactions and/or any medical injuries from food, beverages, wearing costumes, or any activity before, during, or after the party. I, as a result of this release and agree to hold Royal Divas Spa Bus harmless from and waive on behalf of myself, my heirs, and any personal representatives any causes of action, claims, demands, damages, costs, expenses, and compensation for damage or loss to myself and/or property that any act may cause, or that may otherwise arise in any way in connection with any services received from.
